The culture of being defensive.

PUB has done it.  Apple has done it.

In recent events, both had came out strongly to say they were not at fault, only to have reports and actions proving otherwise later.  Is this what they teach top management?

A simplification that turned out to be an obfuscation

He was so resistant to hardcoding the mapping, that he developed a convolted method to simplify it. Then finding out its shortcomings, he started patching it.
Now how am I gonna clean up this mess?
